Cancellation Policy - Poliza de Cancelaciones

Cancellations are accepted up to 24 hours before the scheduled service. Cancellations made within 24 hours of the service, will be charged 50% of the scheduled cost. Appointments cancelled within 4 hours and all no-shows will be charged the full price of the scheduled service booked. By booking an appointment with us you understand and agree to these terms.

NO REFUNDS

We understand that sometimes misinterpretations can happen and sometimes the look you were inspired by may not actually look as you expected, we get it! We never want a client to leave unhappy with their service or look. We strive to make every person leave satisfied with the desire to return in the future. We ask that if for any reason you are not satisfied with your service, please express your concerns to your stylist before you leave the salon. We will be sure to accommodate you and fix the issue. If for any reason there is an issue that arose after you have left the salon please call us as soon as possible to accommodate you. We allow up to 10 days after your appointment for any revisions of haircuts or color. Because this is a service-based business and time is our most valued asset, we can not offer any refunds. We appreciate your understanding.

We pride ourselves on our high level of skill and our long-lasting relationships with our guests. Here are some tips that can make your service results more successful:

Chemical services can have varying results based on your individual hair.
It is extremely important that you make your stylist aware of any and all other processes you have used on your hair.
Your clothes may be exposed to the chemicals and products despite our best efforts to protect them. If you would prefer, you can dress down into a smock to avoid this risk.
There is a risk of your jewelry (earrings and necklaces) being damaged or lost during your service. We recommend you remove them before your service.
Please feel comfortable contacting By-APPT at any time after your service if you have any questions or concerns.

Liability Waiver

I have been made aware by the staff at By-APPT that I am choosing to receive a hair chemical service. I have made my stylist aware of any allergies I have, as well as all the processes I have used on my hair in the past 24 months to assist in their assessment of my hair's current status and the process that I need to achieve my desired results. I realize that this is very important information and that any information I withhold regarding my previous processes will increase my chances of damage and the potential for unpredictable chemical reactions. Chemical processes may cause some damage to the integrity of my hair. There are products recommended to improve the health of my hair as well as maintain the results after the service. I am aware that my stylist has been trained in the service technique and that they will do their absolute best to create the best results and therefore I will not hold liable By-APPT, or my stylist if the process has unexpected or undesired results. Dear valued By-APPT client,

Although every precaution will be taken to ensure your safety and well-being before, during and after your lash extension application, please be aware of the following information and possible risks.

I understand that I will notify By-APPT of any allergies I may have before the use of any products used for either Eyelash Extensions, Eyebrow Tint or Makeup which may be used on me.
I understand that face waxing can result in an allergic reaction or redness. I agree to disclose any known allergies or reactions I have had in the past along with any known allergies I may currently have.
I understand that a full set of lash extensions & tint can make the appearance of my own lashes about 30-50% thicker, and make my lashes appear 20-50% longer.
I understand that lash extension & tint services have some inherent risk of irritation to the orbital eye area, including the eye itself, and could result in stinging and burning, blurry vision and potential blindness should the adhesive enter the eye or should an allergic reaction occur.
I understand that some irritation, itching or burning may occur on the skin if the bonding agent comes into contact with it.
I understand that if the bonding agent comes into contact with my eye, my eye will be flushed with water and I will be assisted in seeking medical attention immediately.
I understand that this is a semi-permanent procedure, as my natural lashes will continue to grow and fall out normally, making touch-up or “fill” appointments necessary to maintain the original look achieved by replacing the lashes that have fallen out. Most clients require a fill appointment every 2-3 weeks.
I understand that while every attempt will be made to provide me with the length and fullness I have chosen, my final result may not be what I initially envisioned.
I have cited all conditions and circumstances regarding my health history, medications being taken, and any past reactions to products or medications.
I understand that additional conditions could occur or be discovered during the procedure which could affect my ability to tolerate the procedure.
I consent to “before and after” photographs for the purpose of documentation, potential advertising and promotional purposes.

I understand that if I have any concerns, I will address these with my lash extension specialist. I give permission to my lash extension specialist to perform the lash extension procedure we have discussed and will hold him/her and his/her staff harmless and nameless from any liability that may result from this treatment. I have accurately answered the questions above, including all known allergies, prescription drugs, or products I am currently ingesting or using topically. I understand my lash extension specialist will take every precaution to minimize or eliminate negative reactions as much as possible. In the event I may have additional questions or concerns regarding my treatment, I will consult the lash extension specialist immediately.

I have read the above information and if I have any concerns, I will address them with my technician/artist. I give permission to my technician/artist to perform the procedure we have discussed and will hold her and the By-APPT staff harmless from any liability that may result from this treatment. I have given an accurate account of the questions asked above including all known allergies or prescription drugs or products I am currently ingesting or using topically. I understand my technician will take every precaution to minimize or eliminate negative reactions as much as possible. I have read and understand the post-treatment home care instructions.

I am willing to follow recommendations made by my technician for a home care regimen that can minimize or eliminate possible negative reactions. In the event that I may have additional questions or concerns regarding my treatment or suggested home product / post-treatment care, I will consult the technician immediately.

I agree that this constitutes full disclosure, and that it supersedes any previous verbal or written disclosures. I certify that I have read, and fully understand the above paragraphs and that I have had sufficient opportunity for discussion to have any questions answered. I understand the procedure and accept the risks. I do not hold the technician/artist, whose signature appears below, responsible for any of my conditions that were present, but not disclosed at the time of this skin care procedure, which may be affected by the treatment performed today.
